PATH—Personal Action Toward Health
This class teaches techniques on how to manage your chronic disease. It is a university tested program with proven results and effectiveness. Participants work on goal setting and effective problem solving along with nutrition, managing symptoms, communication, and relaxation and stress management techniques. This class meets 2 1/2 hours, once per week, for 6 weeks. In addition to a general chronic disease PATH class, special classes are offered with a focus on Chronic Pain Management and Diabetes.
